Referring a patient for nutrition counseling is designed to be straightforward and supportive for both you and your patient.
Step 1: Provide the Patient with the Handout
Share the handout with your patient to help explain nutrition counseling, what to expect, and next steps. This handout encourages the patient to schedule a brief 15-minute consultation discuss their needs, ask questions, and determine next steps. We also go over insurance coverage and steps to what care would look like for your patient.
Step 2: Complete and Fax the Referral Form
